Port Macquarie Disability & Nursing Services is a Registered NDIS Provider - we offer services to clients with NDIS funding - Agency, Self & Plan managed payment options are available.
The NDIS invests in support and services for people under the age of 65, living with a permanent or significant disability. The NDIS is a government funded initiative, which aims to improve long-term outcomes for participants.
The NDIS provides three broad categories of supports for people living with disability:
If you meet the eligibility criteria and you would like to become a participant, you will need to contact the NDIS to make an Access Request. The Access Request form will ask you to provide your personal details, contact information and details about your disability. You can provide copies of existing information, including letters or reports, or you can ask your treating health professional to assist. In some cases the National Disability Insurance Agency (NDIA) may ask you to provide evidence of your disability, this includes information on what your disability is, how long it will last and how it impacts your life. You can work with your treating health professionals to provide your evidence of disability.
Once the NDIA has finalised your access request, they will then send you an ‘Access Decision’ letter to tell you the outcome of their decision. If you’re approved you will work with a NDIS Planner or a Local Area Coordinator to develop your written support plan. The planner will ask questions about how you are going in different areas of your life, what goals you would like to achieve and what kind of help and support you need. This will help them to develop a plan that provides the right support for you.
A Local Area or Support Coordinator will then help you activate your plan. You have a range of options for managing your plan – from self-management through to specialist management by a provider. If you choose to work with a NDIS provider, you will need to make a written Service Agreement with your provider. This agreement will ensure your provider delivers the right supports in the right way.
