What is DVA Community Nursing?

The Department of Veterans Affairs (DVA) Community Nursing Program provides veterans and war widows access to a wonderful range of quality health care and other services.  


Port Macquarie Disability & Nursing Services work closely with DVA to assess, implement and deliver the optimum level of nursing care that you or your family member deserves.  The purpose of the program is to provide community nursing services to eligible individuals in order to meet their clinical and personal care needs. Veterans and war widows who hold a Gold Card are eligible for the service, while veterans with a White Card may also be eligible if the condition needing nursing care is deemed war-related by DVA.  


Our aim for this program is to help maintain the independence, quality of life and health outcomes of veterans and their widows by preventing admission to hospital and or residential aged care facilities by providing exceptional community nursing services and personal care services within the comfort of your own home. 

How can I receive DVA Community Nursing?

       To receive DVA community nursing, the entitled person must obtain a referral from their GP or the Hospital Discharge Planner.  Click below to download the DVA Community Nursing Referral form.


Following this referral, Our DVA Care Coordinator will liaise with you to arrange a convenient time to complete a comprehensive community nursing assessment by a highly experienced and dedicated Registered Nurse.
